All appropriate forms must be completed and forwarded to the Department of Sports Medicine before any athletic participation will be allowed. Forms are printable and may be written or typed. Please mail completed forms and requested documentation to:

Freshmen, Transfers and First-Time Athletes

(If you are not yet 18 years old, a parent or guardian must sign this form.)

• Medical Exceptions Form
  (Student-athletes who take prescribed ADHD/ADD medications (Ritalin, Adderall, etc.) MUST HAVE THIS FORM on file as of the 2012-13 academic year. Failure to have this form on file may affect eligibilty in regard to institutional and NCAA Drug Testing.)
