FORM 1.  ANNUAL INSURANCE FORM (PDF)
A copy of the front and back of your primary insurance card must accompany this signed form!  If you are not yet 18 years old, a parent or guardian must sign this form.

FORM 2.  MEDICAL HISTORY/ PHYSICAL EXAMINATION RELEASE FORM (PDF)

FORM 3. STUDENT HEALTH CENTER PHYSICAL EXAMINATION AND IMMUNIZATION FROM (PDF)
This form is also mailed with your admissions packet.  A physician must perform this physical examination. Completed form must be forwarded to the Student Health Center.
